WILLIAMSBURG, Va. - ChooseHome members, their Personal Services Coordinators and the ChooseHome leadership team recently gathered at the Muscarelle Museum of Art at the College of William and Mary for a summer soiree.
"Coming together as often as we can as a ChooseHome community is one way we can all strengthen our connections and build relationships," said G. Richard Jackson, Executive Director of Riverside's Center for Excellence in Aging and Lifelong Health, which oversees the ChooseHome program. "We talk a lot about the components of successful aging, and being social is one of them. At events like these we are practicing what we preach."
In 2014, Riverside Health System became the first provider of Community-Based Continuing Care (CBCC) in Virginia when it launched ChooseHome, which is available to residents throughout the Williamsburg, Peninsula and Middle Peninsula regions, which include the counties of Gloucester, Mathews, York and James City, as well as Newport News and Hampton.
Also known as life care at home or an aging-in-place program, CBCC programs provide services and benefits to independent older adults who wish to remain in their homes, but want to have access to a safety net of continuing care services and control over the cost of long-term care. Essentially, it's like bringing most of the benefits of living in a retirement community to a personal residence.
ChooseHome is offered to residents 60 and older and brings clients the individualized support they need to remain living in their own home safely and comfortably for as long as possible, possibly for life.
