Dr. Robert Schreiber visits Williamsburg to discuss chronic disease self-management and care transitions

Dr. Robert Schreiber, a geriatrician and professor at Harvard University, recently visited Riverside Doctors' Hospital Williamsburg to meet with leaders from across the state to provide consultations on chronic disease self-management and care transitions.

The meeting included representatives from the Peninsula Agency on Aging, the Virginia Department of Health, Sentara and the Department for Aging and Rehabilitative Services, among others.
